To help patients qualify for Medicare coverage of oral appliance therapy (E0486), please review the following requirements carefully.
1. Initial Evaluation Requirement
Patients must complete an in-person clinical evaluation for obstructive sleep apnea before completing a sleep study.

The evaluation should document symptoms, medical history, and clinical findings consistent with obstructive sleep apnea.

Appropriate ICD-10 diagnosis coding should be included when applicable.

Please note: Traditional Medicare requires an in-person evaluation. Telemedicine evaluations may not meet Medicare guidelines.
2. Sleep Study Requirements
Patients must complete a Medicare-approved sleep study that meets Medicare criteria.
Medicare Sleep Study Criteria

Medicare follows the 4% oxygen desaturation rule.

The sleep study must generally be completed within one year of oral appliance prescription. Many commercial insurance plans may allow a longer timeframe.
The patient must meet ONE of the following:
A. Moderate to Severe Sleep Apnea
AHI or RDI of 15 events/hour or greater with a minimum of 30 events.
B. Mild Sleep Apnea with Symptoms or Comorbidities
AHI or RDI of 5-14 events/hour with at least 10 events, plus one or more of the following:
- Excessive daytime sleepiness
- Mood disturbances or insomnia
Additional qualifying symptoms or comorbidities:
- Hypertension
- Heart disease
- History of stroke
C. Severe Sleep Apnea with CPAP Intolerance
AHI or RDI greater than 30 events/hour, AND:
- The patient is unable to tolerate CPAP therapy, OR
- CPAP therapy is contraindicated and documented appropriately in the chart.
3. Prescription Requirements
After reviewing the sleep study results, the referring provider may prescribe oral appliance therapy.
The prescribing provider does not need to be the same provider who completed the evaluation.
Prescriptions must be:

Signed and dated

Electronically signed or hand-signed

Include the appropriate diagnosis and treatment recommendation
4. Device Delivery & Billing
A licensed dentist (DDS or DMD) trained in dental sleep medicine will provide and bill for the oral appliance.
Important Medicare Compliance Information
Direct-to-consumer oral appliances and mail-order devices may not meet Medicare coverage guidelines or accepted standards of care.
